I think your trouble is because of dates.

So i suggest you to change:

Due date                           - 07/21/2015

Calc. date                         - 07/21/2015

Because you have item effective from, then due date and calc. date have to be filled with the dates for the 1st period of charges - when charges have to take place. The next period will be known because of this dates and Frequenc
